Jаne is the directоr оf а stаte funded cоmmunity health center in a low-in - come neighborhood. Facing another round of budget cuts, she stares at her staffing roster trying to decide who should be included in this round of layoffs. Although it was a year ago, it seemed like only yesterday when she sat at her desk looking at the same roster, only this time the list would need to be shortened by at least 6 names. She could eliminate some manager positions, but would then risk becoming out of compliance with the state regulations. If more staff nurses are cut this time, her already short-staffed facility will need the nurses to work extra shifts, and they are already picking up extras now. Exasperated, Jane thinks to herself, “If I could only show those budget committee members what it is like to practice nursing here! “As a rule, Jane kept her opinions to herself, but the frustration she felt was mounting. “I know that the economy is strained, but I have a hard time keeping qualified nurses on staff here. If I keep piling more responsibility on them, they will burn out, and more than likely, mistakes will be made.” Realizing she could no longer put it off, she had to make people see the conditions her staff were working in, the long hours, and how another round of budget cuts were going to have a negative impact in a community that already had the desk stacked against them. Jane immediately organized a group within the center to discuss the issues and try to figure out how to change the situation.

As usual, the floor was short-staffed, so when it came time to do final tasks before shift change, one of the floor nurses asked Beth to see if Mr. Reynolds or Mr. Renner was supposed to be prepped for surgery. The white board which typically has the most current information had not been updated all evening, so Beth looked in the electronic record. Mr. Renner’s file showed that he was scheduled to be discharged that morning, so Beth told the nurse that it must be Mr. Reynolds. When the nurses were giving report during shift change, Mr. Renner was to be discharged, and Mr. Reynolds was to be prepped for surgery per Beth’s orders. It turned out that Mr. Renner was to be discharged, unless his lab values were abnormal, then the wound would be surgically debrided. Following the instructions given by the previous shift, the day shift nurses directed the nursing aids to pack Mr. Renner’s belongings while they started the discharge paperwork. Unfortunately, Mr. Reynolds was awoken at 6:15 am and was told that they needed to insert a catheter as part of his surgical prep. Luckily, Mr. Reynolds communicated that he was not having surgery, and the mistake was discovered before it escalated. Beth was in the staff lounge when the attending physician entered, telling her that he had just received an incident report from the day shift nurse detailing the issue, and wanted to hear what she had to say. “Nothing happened to the guy. So much for working together and having each other’s back,” Beth thought to herself.
