He has constant indigestion and self-medicates with over-the-counter products. When asked about his family history, he indicates his mother has diabetes and hyperthyroidism. His father has a long history of cardiovascular disease, having had a heart attack at 50 years of age followed by triple by-pass surgery. He is worried that he will be following his father’s footsteps. He also reports having excessive thirst and wakes up several times each night to urinate. He has experienced some dizzy spells at work but doesn’t know what those are all about. Medications: (OTC) (Omeprazole) Prilosec® and Magnesium 400 mg daily. Vital Signs: Blood Pressure: 138/98; Pulse: 110 bpm; Respiration: 20 breaths/minute Dental History: The patient has not had oral care for over 7 years. He claims he brushes with a hard bristled manual toothbrush only if he remembers. He has never flossed and occasionally rinses with an over-the-counter mouth rinse. He has lost several teeth due to gum disease and has been told by previous dentists that he has “bad gums.” He consumes soft drinks and high energy drinks several times daily. He reports no history of smoking cigars or cigarettes. Extraoral Examination: Palpable submandibular lymph nodes were found on the right side. No other abnormalities were detected. Intraoral Examination: A small rounded vesicle was noted in the labial mucosa adjacent to tooth #12. He indicated it was painful and had been there for a few days. Gingival tissues were generally pink and stippled in the maxillary anterior regions, however there were numerous areas of gingival inflammation present in posterior and mandibular areas. Moderate to heavy calculus was visible, particularly in the mandibular anterior teeth. Generalized moderate to heavy bleeding upon probing was present with localized heavy bleeding in the mandibular anterior sextant, and the mandibular right and left posterior molars. Bleeding Index: 75% Plaque Score: 65%
