A 71-year-old right-handed male is brought to the emergency department by his wife after she noticed the right side of his face was drooping, and his speech was slurred while eating breakfast. Symptom onset was approximately 90 minutes ago. On exam: He has right facial droop and right arm/hand weakness (3/5 strength), with the right leg only mildly affected (4+/5 strength). He has decreased sensation to light touch and pinprick on the right face and right arm, with relatively preserved sensation in the right leg. Visual field testing reveals a right homonymous hemianopia  He is unable to follow complex commands and has difficulty naming objects.
