Pоlycystic оvаry syndrоme (PCOS) is the most common endocrine аbnormаlity in women of reproductive age. Although the exact cause is unknown, this condition is linked to insulin resistance and obesity. Insulin is known to help regulate ovarian function, and the ovaries respond to excess insulin by producing androgens, which can lead to menstrual irregularities. Hyperandrogenism is classic in PCOS, causing inhibition of follicular development, ovarian cysts to one or both ovaries, and abnormal menstruation (oligoovulation or anovulation). Body mass index (BMI) c': 30 kg/m2, amenorrhea, acne, hirsutism, acanthosis nigricans, insulin resistance, and infertility are hallmark features of PCOS. Patients with this condition have increased rates of cardiovascular disease, dyslipidemia, and type 2 diabetes mellitus. The incidence of endometrial cancer in patients with PCOS is also much higher due to endometrial hyperplasia from chronic anovulatory cycles. Most patients will have evidence of multiple follicles peripherally located in the ovary (also known as the string of pearls sign), which is a classic ultrasound finding. Transvaginal ultrasounds are preferred over pelvic ultrasounds due to increased visibility of the ovaries and uterus, especially in patients who have an elevated BMI. The 2003 Rotterdam criteria require at least two out of three of the following to make the diagnosis of PCOS: oligo- or anovulation, evidence of hyperandrogenism, and the presence of polycystic ovaries on ultrasound. Multiple treatment approaches are required to treat PCOS. Nonpharmacological approaches to PCOS include exercise and weight reduction, which can decrease androgen and insulin levels. Patients who do not respond to lifestyle changes may benefit from taking metformin, which acts to improve insulin resistance and induce ovulation. Clomiphene is the first-line therapy for infertility to induce ovulation. Aromatase inhibitors also help improve ovarian function and ovulation. Other pharmacological agents include antiandrogens or spironolactone to improve hirsutism and other androgenic effects. In patients who do not desire pregnancy, oral contraceptives are a mainstay of treatment not only to help control hirsutism but also to ensure regular shedding of the endometrium to avoid endometrial hyperplasia. Patients with PCOS should also have lipid profiles and glucose regularly monitored.
