The New England Journal of Medicine, 308 (3):146-151.   Case Study Synopsis:    A 60-year-old man was admitted to the hospital because of confusion and dysphagia (difficulty swallowing).  The patient was well until about four months earlier, when he began making errors at work and was dismissed from his position.  During the two weeks prior to presentation at this hospital he became progressively confused and complained that solid food became stuck in his throat. He was observed to have slurred speech and an unsteady gait. For several days prior to admission he experienced bouts of lower abdominal pain and cramps, increased thirst and increased urination.    Laboratory tests were obtained. Upon examination, the patient’s speech was garbled and he had clumsy neurological responses (e.g., finger-to-nose). Amiodarone (a potassium channel blocker) was administered to reduce heart rate, as were antibiotics.  Dehydration and confusion persisted.  A slow intravenous drip of half normal (0.45%; 154 mOsM) saline was begun.   Imaging of the thorax and abdomen revealed a large mass in the lower right quadrant of the abdomen with significant fluid accumulation.
