Lois is 11 years old. She is in the fifth grade, finishing up school at Tampa Elementary School and will be transitioning to Tampa Middle School in the fall. Lois is a little older than most of her peers because she lost a year due to having to undergo treatment for cancer when she was five years old. The cumulative folder reveals that she went through about three years of treatment and did not start first grade until she was seven years old. She is now on track to complete elementary school and move onto middle school. Lois has recently been referred to the school psychologist (you) by the assistant principal, who has noticed that Lois often spends her free time (e.g., lunch, recess) in isolation. The assistant principal also reported observing that Lois is often the target of bullying. School cafeteria staff report that besides sitting alone at a table, her peers make fun of her size and the way she dresses. The assistant principal notes one thing that is obvious, Lois is clearly obese and shorter for her age compared to her peers. The assistant principal did not note any academic concerns but would like you to follow up on the behavioral and interpersonal issues. A review of Lois’s cumulative folder reveals that she is indeed performing well academically and has been since beginning elementary school. Despite her cancer diagnosis and treatment, Lois never had an individualized education program (IEP). You interview several of Lois’s teachers who report that she is a good student who performs well academically but does not interact much with her peers. They also confirmed the assistant principal’s reports about Lois’s behavior and bullying. Her math teacher, however, states that he observes that recently Lois seems to be distracted in class. In your first meeting with Lois she expresses that she likes school in general but is nervous about leaving her elementary school and her current teachers, and moving onto Tampa Middle School. Regarding her social isolation, Lois expresses a desire to interact with her peers more but states that they do not want to play with her. Lois talks about her favorite classes. She says she loves her English class but not math because she finds some of the concepts confusing and challenging. She has been told by members of her family that girls often don’t do well in math and that she shouldn’t worry about not understanding such concepts. Lois’s mother reports that Lois had a tough time with the cancer treatment, that she relapsed once, but is now cancer free. She was diagnosed with acute lymphoblastic leukemia. She mainly was treated with chemotherapy, but because she had a setback in her treatment, stem cell transplant was also recommended. Lois is now in remission, although she is supposed to go to the hospital for yearly check-ups, appointments which are sometimes hard for her mother to keep. Lois’ mother also reports seeing behaviors similar to what school staff are reporting at home and in community settings and states that Lois has had difficulty maintaining friendships since an early age. Mother also shares that sometimes it is difficult for her to focus only on Lois in the home because she is a single parent as Lois’s father left the house when Lois was first diagnosed with cancer. Lois has very little contact with the father. Lois’s mother works two jobs to try to keep up with rent and food expenses.
