Schaffer, MD ANESTHESIA: General anesthesia. DRAINS: None.  FINDINGS: 40-year-old male suffering with persistent pain in his knee following medial meniscus repair and ACL reconstruction of 1 year. He is taken back to surgery. After appropriate level of anesthesia was achieved the right knee was prepped and draped in an orthopedic manner. We went through the previous portal sites. Sharp dissection was carried through the skin; blunt dissection was carried into the joint space. The patient had some scar tissue in the notch area and we went ahead and debrided this. The hamstring was probed and felt to be intact and functioning nicely. Immediately the patient’s old meniscus repair was not functioning. The patient’s meniscus was a little bit more complexly torn. I went ahead and debrided it. This was an essential bucket handle that would have been previously repaired. We debrided back to stable tissue. We explored the lateral compartment and it was felt to be intact. No loose bodies were noted. We explored the medial and lateral gutters, suprapatellar pouch, and no loose bodies were noted. The articular surface was noted to be in good shape. There was some minimal grade two chondromalacia involving the medial femoral condyle area. The knee was irrigated copiously and 20 cc of 0.25% Marcaine was injected into the joint space and portal sites. We repaired the wounds with interrupted nylon sutures. We dressed the wound sterilely and the patient was placed in a knee immobilizer.
