He does report associated waist pain and abdominal discomfort, but there is no fever, nausea or vomiting. The patient denies a history of sexual activity. He has had no previous surgery. He was not a known hypertensive or diabetic and not a known sickle cell disease patient. Physical exam demonstrates asymmetric swelling of the right testicle with the right testical being somewhat elevated. The right testicle is difficult to fully assess due to edema. Cremasteric reflex on the right side is absent. Based on the likely differential diagnosis, what should your next action be?
