Use this cаse tо cоmplete the fоllowing 3 questions. Terry is а 30-yeаr-old registered nurse, she presents to your private practice with suicidal ideation. Terry presents to your office for the first time with a chief complaint of suicidal ideation that has been increasing over the past month. The impetus for her scheduling the appointment occurred last week when she was driving home from work and imagined herself driving off of an overpass. Terry says she has intermittent thoughts of wanting to go to sleep and never wake up. Terry describes her mood today as “blank” and states that she no longer feels like doing yoga or cooking, activities which she previously enjoyed. She also reports difficulty sleeping at times, stating, “I feel tired all the time but when I lay down, I just cannot go to sleep. I keep seeing all my patients’ faces”. Terry is recently divorced (last year), does not have children and works as an RN on a COVID floor at a large city hospital. She reports a history of depression and anxiety beginning during her senior year of high school. She reports two previous suicide attempts. One, at age 20 when she overdosed on ibuprofen. She reports that her college roommate found her and forced her to purge but did not take Terry to the ED at her insistence. The second time occurred 3 years ago when she cut her wrists. She states her husband found her and she was at a psychiatric hospital for 3 days. She was titrated on Lexapro to 20 mg qDAY which she states she found somewhat helpful; she stopped taking it a year later when she lost her job and health insurance. Terry reports that she drinks wine “most nights” when she comes home from work, around half a bottle. She denies history of drug or cigarette use. She had her appendix removed at age 11 and a history of childhood asthma. No current medications. She reports that she recently threw away her OTC medications due to her past suicide attempt with ibuprofen. She has a bachelor’s degree in nursing. Her mother died when Terry was 8 years old. Terry was initially told that she died of cancer but found out recently that her mother overdosed on a combination of alcohol/benzodiazepines; it is unclear if it was intentional or not. Her father is remarried and has two stepchildren. Terry states “he definitely seems to prefer her kids to me; he says I remind him of my mother.” Terry reports that she has two dogs that she adopted 5 years prior, and they are a source of comfort to her. She reports having close friends in other RNs at work as well as at her church, though she has not been able to attend church since the beginning of the COVID-19 pandemic. Terry speaks and moves slowly during the interview; she becomes tearful when discussing her divorce and the death of her mother. She said, “it’s hard to keep living when I feel like this.”
