(6 points) S: Patient requested to get dressed before heading to the gym for therapy.O: Patient completed supine to sitting edge of bed with partial assist with use of bed rails and bed flat completing log rolling to maintain spinal precautions. Patient required assist with pushing upper body up to sitting with therapist manual assisting at shoulders and hip to assist with sitting up on the edge of bed. Patient completed upper body dressing sitting edge of bed requiring set up of task. Lower body dressing completed sitting at the edge of bed with use of reacher, sock aid and long handle shoehorn, Patient able to donn underwear, pants and socks. Patient completed sit to stand transfer with min assist with use of wheeled walker to pull pants up over hips, and then completed stand pivot transfer to wheelchair at bedside with use of wheeled walker maintaining spinal precautions. Patient brushed teeth and combed hair without assistance from wheelchair. A: Patient required moderate assist for lower body dressing with use of Adaptive equipment during session and would continue to benefit from further practice with assistive technology to donn shoes. Patient completed all transfers with use of wheeled walker and requiring min assist. Patient completed entire session this am maintaining spinal precautions with less than 3 verbal cues.
