A 65-yeаr-оld mаn presents with wоrsening lоw bаck pain over the last three months. The pain began after playing with his grandchildren in their community pool. He describes it as burning, dull, and achy. The pain initially started in his left lower lumbar region but now radiates to his left buttock and posterior leg. He completed six weeks of physical therapy that provided some relief, but the pain returned. He has also been taking daily tylenol (acetaminophen) and as-needed aleve (naproxen)  for the pain, with minimal relief. He denies any weakness, lack of sensation in his lower extremities, or bowel or bladder changes. His past medical history is significant for hypertension and hyperlipidemia. He has not had lumbar surgery. He currently smokes one pack of cigarettes daily. His blood pressure is 140/82 mmHg, heart rate 80/min, respiratory rate 14/min, temperature 98 F (36.7 C), and oxygen saturation 98% on room air. Physical exam consists of rope-like paraspinal muscles that are tender to palpation. Lower extremity strength testing is 5/5 in all major muscle groups bilaterally, lower extremity sensation is intact bilaterally, and deep tendon reflexes are 2/4 bilaterally for the L4, S1 reflexes. Straight leg raise test is positive on the left, with contralateral straight leg raise being positive on the right.
