She is admitted to the hospital and the nurse performs an assessment. Subjective data: Patient reports lethargy, weakness, dizziness, & dry mouth. States too nauseated to eat or drink for 2 days Objective Data: Heart rate 110 beats per minute (normal is 60-100) Blood pressure 84/55 (this is low), Weight loss 5 pounds since last chemo treatment, Dry oral mucous membranes Lab values: Sodium / Na: 131 mEq/L (normal 135-145 mEq/L); Potassium / K: 3.4 mmol/L (normal 3.5 - 5.0 mmol/L)
