The patient noticed swelling two months after she started taking phenytoin (Dilantin) for epilepsy, which was first diagnosed when she was 17 years old. The patient does not brush or floss her teeth regularly and she is also not happy with her smile and would like to get help.Medical history:The patient was diagnosed with epilepsy 14 years ago and has been taking phenytoin (Dilantin) 500 mg daily. There were no other significant medical problems and the patient has no known allergies.Social history:The patient neither smokes nor consumes alcohol.Extra-oral and intra-oral examination:There were no significant findings on extra-oral examination. The patient had no masses or swelling and the temporomandibular joint was within normal limits.Intra-orally, the soft tissues of the oral cavity including the tongue appeared normal. Examination of the gingival tissues revealed generalized marginal erythema, oedema, rolled gingival margins, bleeding on probing, enlarged interdental papillae with gingival margins growing over the clinical crowns especially in the anterior teeth. Probing depths ranged from 2-5 mm all round.
