CEFTAZIDIME (Fortaz) Antibiotic Available- Injection: 500mg, 1gm Pediatric Dosage: 90-100mg/kg/day in 3 divided doses Reconstitution- 500mg reconstitute with 1.5ml of sterile water to yield 280mg/ml 1g reconstitute with 3ml of sterile water to yield 280mg/ml Dilute- Further dilute with NS, D5W, or LR to a final concentration of 7mg/ml for administration. Administer over at least 30-60 minutes.
