Scenаriо 2 A 67-yeаr-оld mаle returns fоr follow up 33 days after starting CPAP for moderate OSA. His initial PSG showed an AHI of 25.3, of which 2.1 were central apneas. The apneas predominated REM sleep. The lowest O2 desaturation was 83%. During a full-night titration, obstructive apneas, hypopneas, and snoring were resolved at a pressure of 11 cmH2O. This included 8 minutes of supine REM sleep. However, notable central apneas emerged, at a rate of 35.3 per hour. Most were at the wake-sleep transition. There was no pattern of Cheyne-Stokes respiration, and the central apneas disappeared during N3 and REM sleep. Sleep efficiency was 47%. The patient reported difficulty adjusting to CPAP. He reported waking up 10-20 times per night most nights. His ESS was only marginally improved from 13 down to 11/24 on CPAP. He reported being anxious about using CPAP and would find himself “breathing fast” as he tried to fall asleep. The nasal mask had caused an open sore on the bridge of his nose. Downloaded PAP data indicated an average CPAP usage of 7 hours and 10 minutes per night with 100% of nights having 4 or more hours of use. However, his mask leak showed a median of 29.5 L/min and 95th percentile of 55.4 L/min. His CPAP download showed an AHI of 17.6, of which 15.2 were central apneas. Medical History: Generalized anxiety disorder Hyperlipidemia Benign prostatic hyperplasia Osteoporosis Medications: Atorvastatin Citalopram Tamsulosin Clinical Findings: BP 123/66 Weight 171 lb. Height 68.5 in. BMI 25.7 Neck circumference = 14.2 in. Mallampati classification IV   Refer to Scenario 2 for questions 6-10. 6.  The diagnostic PSG showed that the patient’s sleep apnea was primarily what type?
