Histоry & Physicаl (H&P): Bаrry is а 61 yо right-handed male whо was found by his spouse at about 4:00 AM, sitting on the couch, unable to stand up, to respond appropriately to questioning, or to speak clearly. The spouse reported that the pt was "speaking funny." He was admitted via the ED and found to have right upper and lower extremity weakness and disorientation. His speech was noted to be very labored, with many pauses and hesitations; intelligibility was estimated at about 5-10%. Past Medical History (PMH): Significant for history of (h/o) left CVA in September 2019, hypertension, high cholesterol, insulin dependent diabetes mellitus type 2 (IDDM-Type 2), and cocaine abuse. Pt noted to have some residual right-sided weakness from the previous stroke that requires the use of a cane, per the spouse. The spouse further reports that pt is non-compliant with medications. IDDM-Type 2 is currently managed by diet. Spouse reports that previous CVA was characterized by (c/b) mild anomia with upper and lower extremity weakness. Social History: The pt lives with his spouse. They have 4 adult children in the area, but have little to no contact with any of them. The pt is unemployed and in the process of applying for disability and has no health insurance. Because of the health insurance situation, the pt did not receive rehab following the first CVA, nor has he seen his primary physician. Education level is high school diploma. Activities of daily living (running laundry, fixing meals or a sandwich, etc.) are limited due to poor motivation. The spouse fixes meals and bathes pt, but pt can do essential tasks like feeding himself and brushing teeth. He also has a significant cocaine habit, smoking approximately once per week for at least 2 years, although spouse reports that pt has been using for much longer. Evaluation: Barry's expressive language skills are the most significantly impaired of his language functions. Expressive language is characterized by many pauses and hesitations, articulatory groping, significant anomia, and frequent circumlocutions and paraphasias. Filler words were largely omitted when attempting to communicate. The pt exhibited a great deal of frustration when responding to questions, often pounding his fists on the exam table. Comprehension was good for simple, concrete language, although comprehension was noted to deteriorate when language became more complex (as when following multi-step directions) or abstract. The pt is able to use gestures and body language such as pointing or taking the person to what he needs, such as a glass of water. Automatic speech tasks are good, but occasionally contain paraphasias. Answer the following questions: 1) What is your clinical diagnosis -- that is, what type of aphasia is most likely present?
