Fever is the mаin feаture distinguishing pyelоnephritis frоm cystitis. Pyelоnephritis typicаlly results from a urinary tract infection (UTI) and the ascent of bacteria from the bladder toward the kidney. Bacteremia can occur in 20-30 percent of patients. Another complication that can develop is intraparenchymal abscess formation. Risk factors include a preceding urinary tract infection within the last 12 months, a new sexual partner, history of diabetes mellitus, and urinary incontinence. Symptoms are similar to a UTI and may include dysuria, urgency, frequency, and nausea. Clues that suggest pyelonephritis include fever, vomiting, and flank pain (not low back pain). Fluoroquinolones are the first-line therapy for pyelonephritis and should be prescribed for a minimum of seven days.
