Hoarseness is due to paradoxical vocal cord movements.  He is a 50 pack year smoker and quit 1 year ago. He also reports having some weight loss, despite any changes in appetite, however he cannot specify how much.  On physical exam there is 2cm firm, non tender anterior cervical lymph node.  What is the most likely diagnosis?
