The paroxysms he experiences are due to intermittent release of catecholamines from the pheochromocytoma. Urinary vanillylmandelic acid (VMA) is a metabolite of catecholamines and is commonly found in pheochromocytoma. The patient also has laboratory evidence of hyperparathyroidism with hypercalcemia, hypophosphatemia, and increased parathyroid hormone levels. With evidence of pheochromocytoma and hyperparathyroidism, this suggests presence of multiple endocrine neoplasia type II (Sipple's syndrome), a rare autosomal dominantly inherited syndrome. MEN type II is characterized by pheochromocytoma, medullary carcinoma of the thyroid, and primary hyperparathyroidism or parathyroid hyperplasia.
