Infectiоus Diаrrheа Diаrrhea is defined as 3 оr mоre loose or watery stools per day. Acute forms are typically due to infectious sources. Pathophysiologically, diarrhea usually develops from a disruption in the small intestine’s absorption and secretion. Approximately 40% of travelers to Asia, Latin America, or Africa get “traveler’s diarrhea.” The most common infectious agents are enterotoxigenic E. coli (ETEC), Salmonella, and Shigella species. Evaluation begins with a detailed stool history, noting the frequency and duration, presence of blood or pus in stool, presence of abdominal pain, recent travel and recent questionable food, water or antibiotic intake. Further evaluation is recommended if red flags are present: fever, elderly, immunosuppression, blood or pus presence, significant pain or severe dehydration. Laboratory evaluation includes serum electrolytes, fecal WBC count, stool and blood cultures, and possibly stool ELISA and pathogen-specific serologies. An abdominal CT may be considered if toxic megacolon is suspected. If no red flags are present, supportive care is the mainstay of treatment, and consists of PO fluids. If severe dehydration is present, oral or intravenous fluid replacement is necessary. Fluoroquinolone antibiotics, such as a 7-day course of ciprofloxacin, are recommended for acute diarrhea which is caused by Shigella.
